To address the elephant in the room immediately: Clive Barker’s Hellraiser: Revival is unapologetically gruesome. Displaying human disfigurement and graphic sexual themes directly through a first-person lens, the game doesn’t just ask you to look—it forces you to reach in and interact with the gore. It translates Clive Barker’s twisted universe into a level of visual fidelity that cinematic adaptations could only dream of achieving. Visually, it is nothing short of breathtaking.

While its provocative shell easily draws attention, writing off Revival as mere shock value misses the point. Beneath the titillating exterior lies a thoughtful exploration of human desire, domestic abuse, and the darkest corners of the psyche. Strip away the blood-soaked spectacle, and you find a title willing to take daring risks with both its mechanical design and storytelling.

Initially, Revival can feel like a mosaic assembled from other famous titles, with each borrowed gameplay concept sewn together like a Frankenstein monster. Yet by the time the credits roll on this 10-hour survival-horror journey, it evolves into a distinct entity. Though it endures a minor identity crisis during its opening chapters, its shameless embrace of grotesque imagery and sensual themes ultimately elevates it into an experience well worth completing.

A Descent Into Hell

Drawing deep inspiration from Barker’s original novella, The Hellbound Heart, alongside the first two feature films, Revival takes place two decades after the events of the original movie. Fortunately, complete newcomers can jump in without needing prior knowledge of the lore. The narrative follows Aidan Lynch and his partner, Sunny—two privileged individuals bonded by their shared pursuit of taboo thrill-seeking through fringe sexual practices and substance abuse. Sunny’s penchant for collecting forbidden occult objects leads her to acquire the Genesis Configuration: a legendary puzzle box capable of tearing open a portal to an endless, concrete labyrinth dimension controlled by the Cenobites—demonic entities dedicated to harvesting human souls for an eternity of agonizing ecstasy.

Unaware of the cube’s supernatural threat, Aidan uses the device during an intimate, consensual moment with Sunny. The ritual inadvertently summons the Cenobites, who brutally butcher Sunny before dragging her into the abyss. Puzzle box in hand, Aidan plunges straight into the underworld to rescue her.

Shortly after arriving, Aidan crosses paths with the Scarlet Church, a bloodthirsty cult devoted to worshipping the Cenobites and their deity, The Leviathan, in hopes of ascending to demonic servitude. However, the cult and hellish monstrosities aren’t his only threats; Aidan is constantly tormented by the specters of his own past mistakes and addiction.

A Tonal and Mechanical Smorgasbord

This narrative setup allows Revival to experiment with a vast assortment of gameplay styles, turning its initial hours into an eclectic sampler platter of game design.

Playing as Aidan, you seamlessly transition from stealth-killing cultists to engaging in brutal, close-quarters melee combat reminiscent of Condemned. Shortly after, you’re firing automatic weapons into groups of monstrosities, flinging circular sawblades, and executing visceral glory kills straight out of Doom (2016)—all while managing a grid-based inventory system inspired by Resident Evil 4. Remarkably, these mechanics feel responsive and satisfying, even if the game occasionally strains to maintain smooth pacing while rushing you between contrasting scenarios.

The level design frequently mimics the compact progression of classic survival-horror mansions, relying on exploration and key-hunting to open new pathways. Rather than sprawling estates, these areas are often scaled down to tight, focused environments: in one section, you gather fetish paraphernalia for a dominatrix in a underground club to unlock a VIP room; in another, you navigate a miniature manor where each unlocked room provides the key to the next. This condensed structure keeps the linear narrative moving at a crisp pace while paying homage to classic Resident Evil level design.

The primary gameplay loop rotates between tactical stealth and intense shootouts against aggressive cultists. The dynamic changes radically when facing hordes of “Unworthy”—bullet-sponge, zombie-like creatures. Encounters with these mobs force you to leverage environmental hazards using the Genesis Configuration, such as luring enemies into traps or utilizing telekinesis to shift massive industrial fan blades across an arena to shred incoming foes.

Initially, the Genesis Configuration functions much like Half-Life 2’s Gravity Gun, letting you grab objects and launch them at targets. However, its interactions are more scripted and situational rather than purely physics-based. Flinging circular sawblades to sever limbs directly echoes Half-Life 2’s iconic Ravenholm level—a nod made explicit by a severed torso pinned to a wall by a saw blade.

As you progress, the Configuration’s capabilities expand dramatically. It gains the ability to absorb fire to throw pyrotechnic attacks, manipulate heavy structures to solve environmental puzzles or crush enemies, and unleash “Hellchains”—an ultimate power that impales groups of enemies with meat hooks and tears them apart. It is endlessly satisfying to execute.

This constant fluctuation between hyper-aggressive action and calculated stealth mirroring the power-exchange dynamics of BDSM culture. Regardless of whether you are playing the aggressor or the prey, the gameplay loop remains consistently engaging.

Combat offers extensive tactical flexibility. For instance, you can grab gasoline cans to pour fuel lines across an arena prior to an ambush, laying elaborate traps for oncoming cultists. Setting up an area with bear traps and fuel lines yields a morbidly satisfying aftermath of dismembered limbs, charred remains, and spent ammunition.

Thankfully, the developers avoided cluttering these systems with unnecessary skill trees or perk menus; combat capabilities evolve naturally alongside the story. When combat needs a breather, The Labyrinth steps in to shift the focus toward environmental traversal and platforming puzzles. While claustrophobic, these segments provide necessary breathing space between firefights, introducing high-stakes chase sequences where Aidan must slide beneath swinging spiked chains.

While effective, these traversal segments can occasionally feel overly linear, serving a somewhat transparent purpose: slowing down the player’s pace while delivering necessary backstory regarding the Cenobites’ origins and motivations.

As the physical world merges with the underworld, any lingering mechanical awkwardness fades away. Cold, brutalist stone structures intertwine with real-world architecture; neon-lit sex clubs bleed into the suffocating grey walls of the maze. Re-visiting earlier locations now draped in chains, living flesh, and vistas of the sprawling labyrinth creates a striking visual atmosphere.

Sensual Themes and Campy Antagonists

The game navigates its depiction of kink and BDSM culture with surprising nuance and care, maintaining a clear distinction between consensual fetish practices and the sadistic violence of the cultists. This boundary is reinforced through environmental details—such as posters promoting consent—and character interactions. In one scene, when Aidan refers to clubgoers as “maniacs,” a merchant named Zora quickly checks him, reminding him that the patrons are merely innocent people seeking an outlet for their desires.

Furthermore, the extensive nudity and explicit content never feel like mindless shock value. Sexual depictions serve deliberate narrative purposes, shedding light on the characters’ inner motivations and personal flaws.

At its core, Revival explores the boundary between love and unhealthy codependency. Aidan’s quest to rescue Sunny forces him to confront his true motivations. When he insists to Zora, “I love her, I need her,” she bluntly counters, “Those are two different things.” This emotional dilemma anchors his character arc straight through to the finale.

Aidan himself strikes a balance between a grounded protagonist and an over-the-top action hero—not unlike Ethan Winters in Resident Evil 7 and Village. He transitions from an ordinary guy to an expert marksman, blade-wielding brawler, and field medic. The narrative justifies his combat proficiency through a colorful backstory: a medical school dropout who joined an outlaw motorcycle gang, learning to shoot and fight along the way. It leans into endearing action-horror tropes while keeping his motivations grounded enough to make his journey genuinely compelling.

The game maintains a balance between serious themes of substance abuse and campy, larger-than-life villains. Bruno, an eccentric “artist” who crafts absurd sculptures out of mutilated corpses, steals every scene he is in. The rogues’ gallery also includes Marlowe, the fanatic leader of the Scarlet Church; the Sister of Mercy; and Charlotte, a voluntary subject of Cenobite experimentation who mutates into a persistent stalker enemy similar to Resident Evil 3’s Nemesis. Each antagonist is given surprising narrative depth, making every boss encounter feel earned.

Among the cast, Zora the merchant stands out as the most compelling figure. Acting as a cynical devil’s advocate, she constantly challenges Aidan’s rationale, forcing him into moments of self-reflection. She serves as a vital narrative anchor alongside Sunny and the Cenobites themselves.

Post-Game Flaws and Final Thoughts

The mechanics and narrative sync up so effectively by the ending that it leaves you wanting to jump right back in for a second run—which makes the absence of a New Game Plus mode particularly disappointing. This omission is further compounded by an unbalanced “Hard” difficulty mode.

Rather than offering a refined challenge, Hard difficulty removes quick-saves and places manual save points far apart. It artificially inflates difficulty by packing rooms with extra enemies who instantly hive-mind to your location upon detection. A single mistake in a lengthy stealth sequence can wipe out 15 minutes of progress, turning skill-based encounters into tedious trial-and-error. Hard mode is difficult to recommend for a standard playthrough unless you are seeking pure masochism.

Despite these minor endgame missteps, Hellraiser: Revival finishes as a confident, self-aware horror experience that fully embraces its unique identity.

In an industry landscape that often shies away from mature, provocative themes, Hellraiser: Revival stands out as a bold statement piece. It pushes sex and violence to surreal, dark-fantasy extremes while treating its core themes of trauma and human connection with surprising thoughtfulness. It is a faithful, worthy expansion of Clive Barker’s legendary legacy.
